Cerebral fat embolism following cardiac surgery.

چکیده
We describe 3 cases of cerebral fat embolism associated with cardiac surgery. The autopsy findings suggested that in 2 cases, the embolic fat globules entered the systemic circulation directly as a result of protrusion of epicardial fat into the left atrial chamber along the suture line in one case, and through an atrial septal defect in the other. The characteristic latent interval between surgical trauma and onset of neurologic manifestations was lacking in both cases. The evolution of neurologic manifestations and the pathologic findings in the third case were similar to those usually seen in posttraumatic fat embolism. This study suggests that the pathogenetic mechanisms of cerebral fat embolism following cardiac surgery may vary in different cases. Also, it emphasizes the importance of autopsy in the investigation of this serious but preventable complication.

Paradoxical Cerebral Fat Embolism in Revision Hip Surgery
The incidence of clinical fat embolism syndrome (FES) is low (<1%) whilst fat embolism (FE) of marrow fat appears to occur more often (Mellor and Soni (2001)). Paradoxical brain FE may occur in patients undergoing hip orthopedic surgery who have an undocumented patent foramen ovale (PFO). We report a case of an eighty-year-old male patient, who underwent a scheduled revision hip surgery sufferi...
متن کاملParadoxical cerebral embolisation. An explanation for fat embolism syndrome.
Fat embolism occurs following fractures of a long bone or arthroplasty. We investigated whether paradoxical embolisation through a venous-to-arterial circulation shunt (v-a) could lead to cerebral embolisation during elective hip or knee arthroplasty. Transcranial Doppler ultrasound (TCD), following the intravenous injection of microbubble contrast, identified the presence of a shunt in 41 pati...

Middle Temporal Vein: A Fatal Hazard in Injection Cosmetic Surgery for Temple Augmentation Because facial aesthetics procedures are trending away from open surgery toward injection surgery, the use of autologous fat has been developed and approved for the correction of facial contour as the newest and most popular dermal filler. Common complications (eg, infections, absorption, asymmetry) are e...
متن کاملFatal fat embolism following an isolated vertebral fracture.
The authors report an unusual case of a fat embolism following an isolated lumbar vertebra wedge fracture. A 66-year-old gentleman sustained a lumbar wedge fracture following a fall. During routine mobilisation some days later he collapsed suddenly and became unresponsive. Shortly after, he suffered a fatal cardiac arrest. A post-mortem was performed which attributed the cause of death to fat e...

There is no specific treatment of fat embolism, whether the embolism is predominantly cerebral, pulmonary, renal or mesenteric. A 46-year-old man had fractured his right femur for the third time in December 1971. Seventy hours later he suddenly developed the cardiovascular, respiratory, a...
